Here is the pool pass application form. Please also review the pool rules and regulations. The following documents must be on file and fees paid in order to receive your pool passes: • Pool Pass Application • Owner Occupancy Form/Vehicle Registration • Pet Registration (if any pet) along with $125 pet fee and current rabies vaccination • Settlement Papers or Lease and Exhibit A for proof of ownership • $125 move-in/out fee

I am a resident at Westmaren in Ashburn, Virginia and am hereby requesting pool passes for the 2010 season. I would like passes for the following individuals that reside in or own the above unit, over the age of 14. I have read the pool restrictions and agree that all names listed above as well as any guests will abide by the Rules and Regulations of the Westmaren Condominium pool. I am also aware that any infringement of the Rules and Regulations may result in my swimming privileges being revoked.
